FORMS DUE AT REGISTRATION:
Girl Health and Medical History Form - All girl members of AHG and tagalongs must complete this form
Adult Health and Medical History Form - All adult members and volunteers who participate in troop activities must complete this form
High Adventure Medical Form - All participants taking part in a "Restricted High Adventure Activity" must complete this form prior to participating in the activity. Restricted high adventure activities include, but are not limited to: boating, canoeing, sailing, cave exploring, downhill skiing, flying, horseback riding, whitewater kayaking, whitewater rafting, rock climbing, rappelling, and challenge courses.
Request for Administration of Medication - Must be completed and returned to troop leader if medication must be dispensed during any AHG troop activity.
Troop Meeting Permission Slip - Must be completed prior to girl participation in any AHG troop activity
Troop Transport Form - Must be completed by anyone transporting troop members
ADDITIONAL FORMS:
At Home Badge Report Form - Record badge work completed outside of troop events
Expense Reimbursement Request- Must be completed to receive reimbursement from the troop budget
KEYs to Child Safety Training - Must be completed by all registered adults.
Swim Test Classification Form - Must be completed by girls and adults prior to participation in any activity that involves water more than ankle deep
Tax Exemption Certificate - Present this any time you are making a purchase for AHG to avoid paying taxes
